I declare that the above information is true to the best of my knowledge and that I am the legal owner of the horse described above. I understand that by signing this form, I agree to surrender legal ownership of my horse listed above to Save a Horse Australia Inc. I further understand that the horse will be adopted out under a lifetime adoption contract to a suitable third party and that I will have no claim or rights to the animal. I shall hold Save a Horse Australia Inc and all its officers, directors, employees and volunteers harmless from any claims of damage, injury or acts of negligence arising from this surrender. I understand that equine activities have a high risk of injury or death. I understand that I may visit the horse listed only by prior appointment. I have thoroughly read and understood this release of liability and agree to abide by it.
